The McCormack Fitness Center is multi-agency facility offering fitness and workout classes to Federal employees.

2023 Annual Membership
Fee Schedule

Membership is from 1/1/2023 to 12/31/2023

Membership Rates are prorated as follows:

  1. $120 - January 1st - December 31st
  2. $90 - April 1st - December 31st
  3. $60 - July 1st - December 31st
  4. $40 - September 1st - December 31st

Renewal Notice

All members will need to fill out the MFC 2023 membership form.

The membership form, including the Health Questionnaire, can now be completed and submitted electronically. Please fill it out completely, and either submit it electronically - using the Submit button at the end of the application - or place in a sealed envelope and slide under the fitness center door. If a NEW member the fee is $10/month. For example, if you join in April your dues will be $10 x 9 = $90.

Note: Dr. Bruce's schedule is every other Wednesday

If you have questions regarding the fitness facility, or fitness programming please contact Dr. Bruce Cohen.

If applicable, please complete the Personal Information Form section.

For people needing a signed Physician's Form, if you have one on file and need another note according to this year's Health History Questionnaire, you have until May 1, 2023 to get a new note to MFC, Inc. For people needing a note for the first time, a doctor's note needs to be on file before we can grant you access. Finally, for those that recently provided a doctor's note since July 1, 2022 (only applies to a few people), no doctor's note will be needed for the one-year 2023 membership.
